How it works

You name what you want first, then make the polite request. 持ち帰りもちかえり (takeaway) is the noun, and で (as) frames it as the way you want your order — "as takeaway." おねがいします (please) is the set polite phrase for asking for something; the します (polite) ending (from する (do)) is what makes it courteous.
