How it works

は (topic) raises 名字みょうじ (surname) as the topic — “as for your surname…”. The question word 何なん (what) fills the slot you want answered, the polite です (is) ties it together, and the final か (question) makes it a question. Word order puts the question word inside the sentence, not up front like English.
