A clipped, polite question. お ((polite prefix)) dresses up 名前 (name) to sound courteous, and は (topic) raises it as the topic — “as for your name…”. The verb and copula are simply left off; the trailing は (topic) plus a rising tone is enough to ask “what is it?”.
