A bare, casual statement of a feeling. が (subject) marks 首 (neck) as the thing that hurts, and the adjective 痛い (hurts) acts as the whole predicate — Japanese needs no "is". For body aches Japanese says "the neck hurts" with が (subject), rather than "I hurt my neck".
